Human review, approval, escalation and audit-grade queue surfaces.

Transactional side effects use @lssm-tech/lib.jobs/outbox/types. The host supplies its transaction and persistent store; capability, approval and caller context rules remain unchanged. Tests use the Jobs in-memory reference adapter.
